To facilitate your first office visit, please bring the following with you:

1. Click on the link at the right to download your intake paperwork. Please fill it out prior to your visit.
2. If you have had any lab work done in the last 18 months, have it faxed to the office or bring a copy with you.
3. Keep a detailed diet diary for at least one week prior to your visit.
4. Please bring all the supplements and medications you take on a regular basis.
   
   

Cancellation Policy

Please be aware that appointments cancelled within 24 hours will be billed half the cost of the scheduled appointment. Thank you for your respect and understanding.

Office Visits

Initial Consultation
Consist of an in-depth patient interview, review of lab work, physical examination and acupuncture treatment as indicated. 60 minutes - $250

Acupuncture Only Intake
One hour intake consultation to begin acupuncture care. Visit includes first acupuncture treatment, but no Naturopathic medical care. You can choose to do a Naturopathic or Nutrition follow up visit to combine therapies in the future. 60 minutes - $150

Follow up Consultations
30 minutes, 45 minutes if acupuncture treatment is also performed.
Naturopathic consult only - $125
Naturopathic consult and acupuncture - $160

Acupuncture Only
Follow up visits last 30 minutes - $90

Phone Consultation New

Phone consultations will include: 1. Intake consultation (Dr. Katahdin will order any labs and make supplement recommendations, she can even sell and ship supplements to you). 2. Follow up phone consultation to review lab work and 3. One final follow up a month later.

Dr. Katahdin will provide each of these at a 15% discount off normal prices ($250 intake, $125 follow ups). **Please note that labs and supplements are separate costs.

Please be aware that payment is expected at the time of service.
The office does not bill insurance companies directly. We will provide the necessary form for you to send to your insurance company for reimbursement. At this time in California, many insurers will reimburse for acupuncture treatment, but not yet for Naturopathic care. You are encouraged to contact your carrier and request Naturopathic coverage in California! First, you need to be informed about your health insurance plan. Read more.
